Lewis's head being popped like a pimple between two weights.

Final Destination 3 - Sultans Gym (Quick Death)

Wendy and Kevin walk into the gym to warn Lewis about Death coming after him. Lewis is currently working out at a bow-flex machine. He notices them come in and exclaims, "Fisher, what the f**k are you doin' here?" Before Lewis gets the chance to react, the weights fall down and crush his head instantly with the bloody scraps splattering all over Wendy and Kevin.
